Position summary 

The Individual Inclusion and Support Officer is responsible for coordinating and developing a responsive service that supports Commonwealth Home Support Programme (CHSP) and Home and Community Care Program for Younger People (HACC PYP) clients to access aged care, disability and community support systems.


The position exercises initiative, professional judgement and delegated authority to coordinate service delivery, manage complex referrals, facilitate social inclusion opportunities and contribute to service growth across the Barwon South West region. The role provides specialist support to clients and carers while fostering collaborative partnerships, promoting service innovation and contributing to the achievement of organisational objectives, funding targets and quality outcomes. The position works within a strength-based, culturally responsive, wellness and reablement framework to improve client independence, social connectedness and access to services under the general direction of the Manager and operates within established organisational policies, program guidelines, delegated authority and approved service-delivery procedures. Complex, unusual or high-risk matters are referred to the Manager where required.

The role is a Permanent Part time position, working 22.5 hours per week.

Duties

  • Coordinate the delivery of individual support and service navigation activities for CHSP and HACC PYP participants.
  • Undertake Service Specific client assessments, care planning, service reviews and outcome monitoring in accordance with funding and organisational requirements.
  • Lead initiatives that promote service growth, innovation and increased participation in individual support and community inclusion programs.
  • Identify emerging community needs and provide recommendations to the Manager regarding potential service improvements, participation opportunities and program development.
  • Act as a key operational contact and provide service-specific information and guidance to clients, carers, community agencies and internal stakeholders within established program guidelines.
  • Provide service-related information, guidance and day-to-day support to colleagues, volunteers and students, without formal staff-management responsibility unless specifically delegated.
  • Monitor service performance and contribute to the achievement of funded activity targets and organisational objectives.
  • Ensure all service delivery activities comply with CHSP Program Manual 2025-2027, HACC PYP Guidelines, organisational policies and legislative requirements.
  • Utilise My Aged Care to manage client referrals, including acceptance, prioritisation, allocation and coordination of services.
  • Maintain accurate and timely records within Carelink+, DEX, My Aged Care, RRHACS Repository and other required systems.

Qualifications and Experience

  • Diploma of Community Services, Ageing Support, Disability, Social Work or a related discipline, or demonstrated equivalent experience in community services.
  • Current Full Victorian drivers licence.
  • Demonstrated experience coordinating services within aged care, disability, community health or social support settings.
  • Experience undertaking assessments, care planning, service reviews and service coordination.
  • Demonstrated experience building and maintaining effective stakeholder partnerships.
  • Experience monitoring service outcomes and contributing to quality improvement initiatives.
  • Sound knowledge of CHSP, HACC PYP, My Aged Care, NDIS and community service systems.
  • Understanding of current Aged Care reforms and wellness and reablement principles.
  • Knowledge of compliance, reporting and quality standards applicable to funded community service programs.

Desirable:

  • Lived experience as a member of a multicultural community.
