Position summary
The Care Manager is responsible for delivering high-quality care management to participants receiving services through the Support at Home program. This includes coordinating participant-centred care, managing service delivery and budgets, supporting participant choice and independence, and contributing to program quality, growth, and continuous improvement.
The role is a permanent part time position (30.4 hours per week).
Duties
- Manage a caseload of home-based participants, ensuring high-quality, individualised care planning and service delivery.
- Monitor participants’ changing needs and respond promptly to ensure supports remain appropriate.
- Ensure initial and ongoing care plans align with participant goals and promote independence, wellbeing, and reablement.
- Support participants to make informed decisions, including respecting their right to take risks in line with the supported decision-making framework.
- Develop, manage, and review participant budgets on a quarterly basis, or more frequently where care needs change.
- Monitor package expenditure and contribute to program budget management to ensure at least 90% of available funds are utilised by the end of each quarter.
Qualifications and Experience
Essential
- Formal qualification in relevant area of health, e.g. Diploma of Nursing, Cert. III Individual Support (Ageing), Cert. IV Aged Care, Diploma Community Services (Case Management.
- Have minimum 3 years’ recent experience managing/delivering direct care/home-based care services to elderly individuals.
- Have demonstrated expertise in care assessment and planning and individualised care.
- Have clear written and verbal communication skills, including computer literacy (including keyboard skills, ability to operate and adapt to care packages, effective internet usage).
- Hold a current and valid Victorian driving licence.
Desirable
- Have recent experience in, and awareness of, contemporary issues in residential and community aged care, including Commonwealth Aged Care Assessment, residential care, community care packages, dementia care.
- Have demonstrated expertise in budgeting and quality reporting.
- Understand local community services available to home-based clients that may be used to enhance their life quality and maintain community connectedness.
- Have an understanding of the migrant experience, with empathy and sensitivity to the needs of the elderly from different cultural backgrounds.
- Proficiency in languages other than English will be highly regarded.
